Shot Gun Start – 7:30 a.m. (please arrive by 6:45)

1. The entry fee is $110 individually or $400 for a foursome. The fee includes 18 holes of golf, a cart, range balls, prizes, and lunch.

2. All coaches, their families, guests, and those who support the AHSACHOF are welcome to participate. 

3. You may enter as a single, two, three, or foursome.

4. All entries are on a ‘first come—first served’ basis. We are restricted to 36 foursomes, so act soon to secure your team’s spot! 

5. For those who wish to be cart-mates, please complete the enclosure.        

6. If you plan on attending the social only, please remit a check for $25.00

7. If you cannot join us, please consider donating to AHSACHOF or sponsoring all or part of a hole.  Hole sponsorship is $100.  A golf foursome plus a hole sponsorship is $450.

8. Make checks payable to; AHSACHOF, and send your completed entry form and check(s).  Please send ONE or ALL checks in a single envelope.
